Men’s Club Playoff Paperwork

All clubs entering the Midwest Spring playoffs will be required to complete and have copies of the following paperwork.

1. Club Roster Check List

Clubs may carry a maximum Roster of 28 players per Event Weekend and a maximum Match Roster of 22 players and up to five (5) non-resident players are permitted on this Roster. For the purposes of this event, a non-resident is defined as person who is not a U.S. citizen and does not possess permanent or conditional resident alien status at the time of the competition.

2. Club CIPP List  (Print a Copy from www.usarugby.org and have with you at check in)

a)  Player must be CIPP enrolled for the club, through initial enrollment or transfer, prior to the earlier of
 (i) playing in a Qualifying Match or (ii) April 15th for men’s competition.

b) The player must not have played in a Qualifying Match for any other Club during the Fifteens  Competitive Season.
 
c)        Non-resident players must be in the United States by March 15th for senior men.
 
d)        Player must be at least 18 years of age unless granted a waiver by USA Rugby Eligibility Committee.

3. Proof of Citizenship for each player

Acceptable forms of proof include
a)  Copy of Passport Picture Page.
b)  Copy of Birth Certificate
c)  Copy of Resident Alien Card (Green Card)

Any player that cannot provide one of the above will be considered a non resident. A Passport Stamp will be used to verify arrival date in country to illustrate eligibility for Non Resident/Alien players.

4. Signed Championship and Waiver Release Form

Each Player must have a sign at team check in. If participant is less than 18 years of age, a signature of parent or legal guardian is required. All Players must affirm that they have the minimum medical insurance coverage in Section 4.
CLUBS SHOULD PRODUCE A PLAYOFF BINDER TO HOLD ALL ABOVE INFORMATION AND FORMS. PLEASE ORGANIZE IN SAME ALPHABETICAL ORDER AS CLUB ROSTER CHECK LIST.  THIS BINDER WILL BE PROVIDED AT THE MIDWEST CHAMPIONSHIPS CHECK IN. SHOULD YOUR CLUB ADVANCE TO THE USA PLAYOFFS, THIS BINDER SHOULD BE CARRIED WITH YOU TO PROVIDE PROOF OF ELIGIBILITY IN THE EVENT OF A CHALLENGE.
It is also suggested that you carry all match reports for your 2009 Fall League Matches. This will help support your club and all your player’s eligibility in the event of an eligibility challenge.
